Q: What should I expect during my first visit?
A: Please arrive early to your appointment so you will have time to fill out necessary paperwork. Your evaluation will last 30 minutes.
Q: What should I expect during subsequent visits?
A: Treatment session time will be used to implement both the structural and functional components of your treatment process. Gym exercises are a common part of PT sessions.
Q: What should I bring to my first visit?
A: Please bring:
- Proof of identity (such as a driver’s license)
- Insurance card
- Adequate child care for your children if they come along
- Completed Crossroads Adult Medical History Form or Crossroads Pediatric Medical History Form and “Welcome to Crossroads” New Patient Form.
- A prescription for physical therapy if you have been given one by your doctor (please note that a prescription is not necessary prior to your first physical therapy visit, but if you have one, please bring it along with you)
Q: What should I wear?
A: You should wear appropriate exercise attire including loose-fitting, comfortable clothes and sneakers.
Q: If I am running late should I still come to my appointment?
A: Please call the office (1-860-228-0194) in this situation for further instructions
Q: What if I am unable to make it to my appointment?
A: Please call the office (1-860-228-0194) to cancel your appointment. Our cancellation policy is that if you do not cancel 24 hours prior to your appointment time, you will be charged a $45 cancellation fee.

Q: Can I continue to use the gym once I am discharged from Physical Therapy?
A: Yes, you may utilize the gym for a modest fee. Please speak to the office staff for complete details.
